This guide offers Medicaid directors, policy leads, and eligibility staff operational guidance for using existing administrative data to identify individuals with substance use disorders or disabling mental conditions who qualify for an exemption from community engagement requirements.

Medicaid is the nation’s largest payer for mental health and substance use treatment, covering nearly one-third of adults with mental health conditions and one-fifth with substance use disorders. In July 2025, Congress passed H.R. 1, making major changes that could leave millions without access to mental healthcare. This report provides states with the information they need to respond to these changes and protect access to care.
Medicaid pays for 25% of all mental healthcare and 40% of all substance use treatment in the United States, making it a critical part of the nation’s care system. Without Medicaid, many mental health and substance use conditions would go untreated, placing strain on individuals, families, communities, hospitals, and state budgets. This 50-state report details how proposed Medicaid cuts could impact access to mental health services nationwide.
A report from Count on Mothers and Inseparable reveals that most families struggle to access and afford children’s mental healthcare. More than half of mothers worry about their children’s mental health, and nearly one in four who seek help cannot get it due to cost or long wait times. Mothers identify expanding school-based supports, fixing insurance failures, and making the system easier to navigate as key solutions.
Recent data show that investments in school-based mental health supports are making a positive difference for young people. However, more work is needed to ensure every school has effective programs that promote mental well-being and provide services for students who need them. This report highlights the importance of continuing to expand and strengthen these supports.
States across the country have made significant progress in improving mental health crisis services in recent years, but more work remains. This report outlines policy solutions to ensure people in crisis receive the right services at the right time, youth and families have developmentally appropriate supports, and law enforcement involvement becomes the exception rather than the rule. The goal is a crisis response system that promotes recovery and support.
